Stopping a GLP-1 injection suddenly is not dangerous for most patients, although appetite can return within weeks as the medication leaves the body. We recommend a planned step down agreed with your endocrinologist. Your endocrinologist can lower the dose in steps and lengthen the time between injections, which may allow appetite to return more gradually. There is no single correct schedule, because the pace depends on your response and your circumstances.

Coming off a weight loss injection is a decision your endocrinologist will plan with you. Our article on what happens when you stop taking Mounjaro, Ozempic or Foundayo covers the changes in the body afterwards. Our article on the maintenance dose covers the dose you settle on while still on treatment, and our article on keeping the weight off covers life after the last dose.
Can you stop a weight loss injection suddenly?
For most patients, stopping suddenly is not dangerous in itself. GLP-1 medications such as Mounjaro, Wegovy and Ozempic leave the body over a few weeks, and the effects on appetite fade over the same period. The same applies to the oral options, the Wegovy pill and Foundayo.
We still do not recommend a sudden stop. Appetite can return quickly once the medication has cleared, before new eating habits have been tested without it. The British Heart Foundation describes weight regain as common after stopping. A planned step down gives you and your endocrinologist time to see how your appetite and your weight respond at each stage.
What if you take the medication for type 2 diabetes?
If you take Mounjaro, Ozempic or another GLP-1 medication for type 2 diabetes, stopping it can worsen your blood sugar control. In that case, your endocrinologist will plan an alternative treatment before you stop and will review your blood sugar after the change. We recommend that you do not stop a diabetes medication without a plan agreed with your endocrinologist.
What does a gradual stop look like?
A gradual stop has two parts. Your endocrinologist will decide each step with you at a follow-up. A gradual step down is Endocare’s clinical practice, and its benefit over stopping at once has not been established in clinical trials. The aim is to let appetite return more gradually, so that you test the eating pattern you built during treatment while some support is still in place.
Lowering the dose in steps
Your endocrinologist can lower the dose one step at a time, holding each step for as long as your response suggests. Dose decisions are made by your endocrinologist together with you at the monthly follow-up, in roughly four-week increments, based on your weight loss response, how well you tolerate the medication and your body composition trends. There is no automatic increase after four weeks. Some patients are already on a low maintenance dose when it is time to come off the medication, so the step down may be short.
Lengthening the time between injections
Your endocrinologist can also lengthen the time between injections before stopping. Your endocrinologist will decide each change from how your appetite and your weight respond, and will review it at each follow-up.
There is no fixed schedule of dose reductions and intervals, because the right pace depends on your response, your starting dose, how long you have been on treatment and your circumstances.
What should you expect as the medication leaves your system?
Appetite returns as the medication leaves your system. Harvard Health Publishing describes increased hunger and a return of thoughts about food in the weeks after stopping, and advises planning meals around protein, fibre and regular eating times to manage it. This is expected, and it is the reason the step down is gradual.
Some patients regain weight. The British Heart Foundation reports an analysis of SURMOUNT-4, a clinical trial in which participants who had lost at least 10% of their body weight on tirzepatide were switched to a placebo injection for a year. In that group, 82% regained at least a quarter of the weight they had lost. That figure describes outcomes without structured maintenance support. Endocare’s maintenance phase is built specifically to keep patients well below it. Regain is possible if the underlying causes of weight gain are not addressed. Some patients regain some weight even when they keep the habits they built, and staying in contact with the clinic allows a small increase to be acted on early.
What should you have in place before you stop?
The step down starts with a plan. Before the dose is lowered, we recommend having the following in place:
- A maintenance plan for food, protein and activity agreed with your lifestyle coach
- A follow-up date with your endocrinologist after the last dose
- A recent body composition analysis as your reference point
- A plan for the first weeks of stronger appetite
- Agreement on when you would return for review if weight rises
Our lifestyle coaches work through what to expect with you before the dose is lowered, including how to handle stronger cravings and which habits to keep once you stop taking the medication. Many patients find that the eating pattern they built during treatment, with fewer refined carbohydrates and less added sugar, makes appetite easier to manage than they feared. Endocare’s lifestyle coaches continue after the last dose.
What if you have to stop because of cost or supply?
Some patients consider stopping because of cost before they have reached their goal. We recommend raising this with your endocrinologist before stopping. Options may include a lower maintenance dose, a longer interval between injections, or a different medication, and your endocrinologist will discuss which of these is appropriate for you. Our pricing page sets out the programme and consultation options.
Supply interruptions are handled the same way. If your usual medication is unavailable, we recommend contacting the clinic before missing a dose. Our article on buying Mounjaro online in the UAE explains why unlicensed supply is a risk.
Can you take a break and restart later?
A break is possible, and restarting begins with a reassessment. The US prescribing information for Wegovy advises that after more than two consecutive missed doses, treatment is resumed as scheduled or re-initiated by following the dose escalation schedule again, which may reduce gastrointestinal side effects. Your endocrinologist will decide the restarting dose from how long the break was and how you responded before.
Some patients return for a short re-engagement. If your weight starts rising after treatment stops, contact your endocrinologist early. They can review your lifestyle plan and whether further treatment is appropriate.
How does Endocare manage the step down?
At Endocare, patients slowly reduce the dose or lengthen the time between injections as advised by their doctor, continue lifestyle coaching, and stay in contact with the clinic. Monthly follow-up appointments can be held in person at the clinic or online. We recommend the first consultation in person, as it includes a physical examination and body composition analysis.
At Endocare, body composition analysis is included free of charge at every consultation. Body composition analysis estimates body fat, muscle mass and body water. Repeated measurements help your endocrinologist understand how your body is changing beyond the number on the scale. During the step down, repeated measurements help your endocrinologist see how your body composition is changing. Your endocrinologist will use that trend to decide the next step. Patients should not decide by themselves to start a GLP-1 medication. A doctor’s review is critical to ensure the treatment is safe and appropriate for the individual.
Frequently asked questions
Can I stop taking Mounjaro suddenly?
You can, and it is not dangerous for most patients, but we do not recommend it. Appetite can return quickly once the medication has cleared, and a planned step down with your endocrinologist gives you time to adjust at each stage.
How do you step down safely?
A safe step down involves lowering the dose in steps and lengthening the time between injections, with each change decided by your endocrinologist at a follow-up from how your appetite and your weight respond. There is no single correct schedule.
Will my appetite come back straight away?
Appetite may return more gradually when the dose is lowered in steps, although the pace varies between patients. Harvard Health Publishing describes increased hunger and a return of thoughts about food in the weeks after stopping. A gradual stop is intended to let this happen in stages.
Will I regain the weight?
Some patients do, and regain is not inevitable. The trial figures most often quoted describe outcomes without structured maintenance support. Endocare’s maintenance phase provides continued coaching, a protein target, resistance training and monthly contact with the clinic, and it is built to keep patients well below those figures.
Can I take a break and restart later?
A break is possible, with a reassessment by your endocrinologist. Restarting after a longer break often means following the dose escalation schedule again from a lower dose. Your endocrinologist will decide with you whether further treatment is appropriate.
What if I have to stop because of cost?
We recommend raising it with your endocrinologist before stopping. A lower maintenance dose, a longer interval between injections or a different medication may be options, and the step down can be planned in the same way as for any other patient.
When it is time to come off the medication, the first step is a follow-up with your endocrinologist. Your endocrinologist will review your weight, your body composition analysis and your reasons for stopping, and will agree the step down and the maintenance plan with you.
