Quick answer

Appetite returns when a GLP-1 medication is stopped, and some patients regain weight. Regain is possible if the underlying causes of weight gain are not addressed. Lifestyle coaching is designed to address them. The trial figures most often quoted describe outcomes without structured maintenance support, so they are not a prediction for a patient who keeps that support in place. Protein, resistance training, continued monitoring and early contact with the clinic can help to protect the result.

Key takeaways
  • Appetite returns after stopping a GLP-1 medication, and this is expected. Maintenance is planned before the last dose for that reason.
  • The British Heart Foundation reports an analysis of the SURMOUNT-4 trial in which 82% of participants switched to placebo regained at least a quarter of the weight they had lost within a year.
  • Those trial figures describe outcomes without structured maintenance support, and they are not the expected outcome for a patient who stays in contact with the clinic.
  • Regain is possible if the underlying causes of weight gain are not addressed, and it is never simply a matter of willpower.
  • A review in the journal Nutrients concluded that higher protein intakes help protect fat-free and skeletal muscle mass during weight loss, either on their own or together with exercise.
  • Resistance training was the only form of exercise associated with an increase in fat-free mass in a study of 304 adults published in Frontiers in Endocrinology.
  • At Endocare, patients slowly reduce the dose or lengthen the time between injections as advised by their doctor, continue lifestyle coaching, and stay in contact with the clinic.
In one line: Weight regain after a GLP-1 medication is not inevitable, and Endocare’s maintenance phase continues coaching, monitoring and contact with the clinic after the last dose.

Patients approaching the end of treatment, and people deciding whether to start, often ask how to keep the weight off after a GLP-1 medication. Our article on coming off the medication safely explains how your endocrinologist reduces the dose, and our article on the maintenance dose covers the dose you settle on while still on treatment.

What happens to your body when treatment ends?

GLP-1 medications such as Mounjaro, Wegovy and Foundayo reduce appetite and food intake by acting on the same receptor as a natural gut hormone. When the medication stops, that effect fades over a few weeks and appetite returns towards its previous level. Harvard Health Publishing describes increased hunger and a return of thoughts about food in the weeks after stopping.

Resting metabolic rate also falls after weight loss. A study in the International Journal of Obesity measured a decrease in resting metabolic rate of around 101 calories a day in adults who lost around 8 kg over a year. It attributed roughly 60% of that decrease to the loss of energy-expending tissue and roughly 40% to metabolic adaptation. A lower resting metabolic rate means that returning to the portions you ate before treatment would lead to weight gain. The eating habits built during treatment therefore remain important after it ends. Our article on what happens when you stop taking Mounjaro, Ozempic or Foundayo covers these changes in more detail.

What do the trial regain figures measure?

The figures most often quoted come from trials in which participants who had lost weight on the medication were switched to a placebo injection and followed for a year. The British Heart Foundation reports an analysis of SURMOUNT-4, a clinical trial of tirzepatide. In an analysis of the SURMOUNT-4 trial, 82% of participants who stopped tirzepatide regained at least a quarter of the weight they had lost. Around 24% regained three quarters or more, and 9% regained all of it.

Those figures describe outcomes without structured maintenance support. In SURMOUNT-4, a clinical trial of tirzepatide, participants switched from tirzepatide to a placebo injection at once, and the medication was not reduced gradually or restarted when weight began to rise. Endocare’s maintenance phase is built specifically to keep patients well below those figures.

Why is regain possible but not inevitable?

Regain is possible if the underlying causes of weight gain are not addressed. Lifestyle coaching is designed to address them, including appetite, eating patterns, activity, sleep and stress. Many patients eat fewer refined carbohydrates and less added sugar by the end of treatment, and this can make cravings after stopping less intense than they expect.

Regain is never simply a matter of willpower. Some patients regain some weight even when they keep the habits they built, because life circumstances change. The aim is to stay close enough to the plan and to the clinic that a small increase does not become a large one. Where weight starts to rise, we recommend contacting your endocrinologist early. Your endocrinologist can review your lifestyle plan and whether further treatment is appropriate.

What protects your result?

Protein

A review in the journal Nutrients concluded that higher protein intakes help protect fat-free mass and skeletal muscle mass during weight loss, either on their own or together with exercise. After treatment ends, protein also supports fullness at each meal, which matters once the medication is no longer reducing appetite. General guidance such as the International Society of Sports Nutrition suggests roughly 1.2 to 1.6 g of protein per kg of body weight per day for people losing weight. Endocare’s lifestyle coaches will personalise that figure from your body composition analysis.

Resistance training

A retrospective study of 304 adults on a calorie-restricted diet, published in Frontiers in Endocrinology, found that resistance training produced the greatest reduction in fat mass and was the only form of exercise associated with an increase in fat-free mass. Preserving muscle remains important for strength, physical function and maintaining a healthier body composition. Two to three sessions a week can help. Our article on resistance training while losing weight explains how to start without a gym.

Continued monitoring

Short-term changes on the scale are often influenced by body water, alongside normal day-to-day variation, so a single reading after the last dose does not tell you whether weight is returning. At Endocare, body composition analysis is included free of charge at every consultation. Body composition analysis estimates body fat, muscle mass and body water. Repeated measurements help your endocrinologist understand how your body is changing beyond the number on the scale. Continuing those reviews after the last dose allows a small change to be acted on early.

Is it worth starting if I might regain the weight?

For many patients, yes, particularly where the aim is a loss of 10 to 15% of body weight or more. Weight loss of this magnitude is often associated with clinically meaningful improvements in blood pressure, lipids and blood sugar, and some patients with prediabetes may return to the normal glucose range. Those improvements are gained during treatment. The aim of maintenance is to keep as much of them as possible.

The decision depends on your individual assessment. Weight loss medication is generally considered for patients with a BMI of 30 or above, or 27 or above when weight-related health conditions are present. Your endocrinologist will assess whether medication is appropriate based on your individual health and weight profile. Patients should not decide by themselves to start a GLP-1 medication. A doctor’s review is critical to ensure the treatment is safe and appropriate for the individual.

What does maintenance look like at Endocare?

At Endocare, patients slowly reduce the dose or lengthen the time between injections as advised by their doctor, continue lifestyle coaching, and stay in contact with the clinic. Before the dose is lowered, our lifestyle coaches work through what to expect with you, including how to handle stronger cravings and which habits to keep once you stop taking the medication.

After the last dose, patients stay in contact with the clinic and can return for a short re-engagement if weight starts to rise. Monthly follow-up appointments can be held in person at the clinic or online. We recommend the first consultation in person, as it includes a physical examination and body composition analysis. Our endocrinologists and lifestyle coaches have treated more than 2,000 people in the UAE through the programme. Maintenance is part of the programme from the start.

Frequently asked questions

Will I regain the weight after stopping?

Some patients regain some weight, and regain is not inevitable. The trial figures most often quoted describe outcomes without structured maintenance support. Continued coaching, a protein target, resistance training and monthly contact with the clinic can help to protect the result.

Do I have to stay on the medication for life?

Not necessarily. Some patients come off the medication after reaching their target weight and maintain their result with lifestyle support. Others stay on a low maintenance dose for longer, or return for a review if weight starts to rise, where your endocrinologist will decide whether further treatment is appropriate. Your endocrinologist will decide with you at each monthly follow-up.

What do the trial regain figures show?

They show what happens when the medication is replaced by a placebo with no structured maintenance support. In the SURMOUNT-4 trial analysis reported by the British Heart Foundation, 82% of participants switched to placebo regained at least a quarter of the weight they had lost within a year. The figure is not a prediction for a patient who keeps coaching and monitoring in place.

What protects my result the most?

No single factor protects the result on its own. Adequate protein, resistance training, continued monitoring with body composition analysis and early contact with the clinic work together. Endocare’s maintenance phase includes all of them.

Is it worth starting if I might regain it?

For many patients, yes. The health improvements associated with weight loss are gained during treatment, and structured maintenance is designed to keep them. The decision depends on your individual assessment with an endocrinologist.

How long does maintenance support last?

Patients stay in contact with the clinic after the last dose, and the length of that contact is agreed with your endocrinologist from how your weight and your body composition respond. Some patients return for a short re-engagement months later, and this is part of the plan.

Talk to an endocrinologist about maintenance

If you are approaching the end of treatment, or deciding whether to start, ask what maintenance looks like at Endocare. Your endocrinologist will review your weight, your body composition analysis and your eating and activity habits, and will agree a maintenance plan with you before the last dose.

You can talk to an endocrinologist at our clinics in Dubai and Abu Dhabi.
